A Research and Biomedical Hotpot: Cell and Gene Therapy

With the advancement of science and medicine, an increasing number of techniques have been developed to enable therapies at cell and gene levels. As of 2025, the United States has approved almost 50 cell and gene therapies (CGTs) for the treatment of serious, life-threatening diseases, and many more are awaiting approval, with more than 4000 currently in clinical or preclinical development[1], [2]. Among these therapies, chimeric antigen receptor (CAR)-T cell therapy has emerged as one of the most effective strategies and a leading area of research in recent years, which is thought to revolutionize cancer immunotherapy. CARs are designed to target tumor associated antigens and activate T cells to specifically kill tumor cells. This strategy has demonstrated strong efficacy against hematologic cancers, including CD19-targeting CAR-T cells and BCMA-targeting CAR-T cells[3]. Beyond targeting on hematologic malignancies, numerous trials are ongoing to explore applications in solid tumors, autoimmunity, and senescence-associated diseases, HIV/AIDS. In addition to CAR-T therapy, other T cell-based strategies are also explored, such as TCR-T (engineered T cells expressing tumor-specific T cell receptors) and tumor-infiltrating lymphocyte (TIL) therapy. Similarly, beyond T cells, the engineering of CAR is also applied to other immune cells, such as CAR-NK and CAR-M(macrophage), to investigate more potentials of using the cytotoxic ability of immune cells.
